Cardiac Specialist Nurse

We are seeking a dedicated and motivated Cardiac Specialist Nurse to join our team at Birmingham Community Healthcare NHS Foundation Trust. As a key member of our Cardiac Services Heart Failure Team, you will play a vital role in providing high-quality patient care and contributing to the development of our cardiac services.
Key Responsibilities:
- Assess, plan, deliver, and evaluate patient-centered care, including medication optimization, symptom management, and education to improve quality of life.
- Work in partnership with patients, carers, and other healthcare professionals to promote health and well-being.
- Provide care for individuals requiring management of their heart failure, including support in their own homes, community clinics, cardiac venues, and virtual appointments.
Requirements:
- Significant experience of working within a cardiac environment.
- Understanding of the needs of socially and culturally excluded groups.
- Experience and understanding of the NHS and healthcare systems.
- Significant experience in delivering community-based cardiac care.
Skills and Knowledge:
- Excellent verbal, written communication, and IT skills, including Microsoft Office Tools and Patient Clinical systems.
- Ability to produce high-quality, error-free clinical documentation and prepare presentations in a timely manner.
- Good planning and organization skills, demonstrating an ability to work under pressure.
- Good time management, planning, and prioritization skills.
Personal Qualities:
- Ability to work professionally, confidently, and effectively as part of a team.
- Willingness to be flexible to meet the needs of the service, including delivering Heart Failure and Cardiac Rehabilitation at different locations across the trust at short notice.
Other Requirements:
- Ability to independently travel around the trust at short notice to enable delivery of the current service contract and to meet service needs. Car driver essential.
