Job Description

We are seeking a dedicated and compassionate Care Coordinator to join our team at NELFT North East London Foundation Trust. This is an exciting opportunity to work with looked after children and make a positive impact on their lives.

Salary

The estimated salary for this position is £35,000 - £45,000 per annum, depending on experience.

Key Responsibilities

As a Care Coordinator, you will be responsible for:

  • Providing a nursing service to support looked after children in achieving optimal health outcomes
  • Working effectively across health, social care, education, and the voluntary sector to achieve the best health outcomes for looked after children
  • Delivering review health assessments for children and ensuring effective implementation of health care plans
  • Improving the coordination, access, and delivery of healthcare to children and young people in care (0-19 years)
Required Skills and Qualifications

To be successful in this role, you will need:

  • Nursing or midwifery qualifications and registration with the NMC
  • SCPHN HV/SN qualification
  • Minimum 3 years post-registration nursing experience
  • Experience of working with children
  • Knowledge and experience of safeguarding issues
  • Training experience
