Job Title: Advanced Pharmacist

We are seeking an experienced Advanced Pharmacist to join our team at Blue Sky Marketing. As a key member of our organization, you will play a vital role in improving the provision of medication for discharge and enhancing patient flow.

This is an exciting opportunity to work with our teams at Basingstoke and North Hampshire Hospital and the Royal Hampshire County Hospital in Winchester. You will have the chance to develop your clinical and leadership skills, as well as gain experience in project management.

About the Role:

  • To support the effective transition of patients from secondary care into primary care providers.
  • To facilitate the safe and efficient supply of discharge medication for patients within secondary care, enhancing patient flow.
  • To support the lead transitional care pharmacist in the management of the wider transitional care team on a day-to-day basis.
  • The post holder has joint responsibility with other pharmacists within the clinical team for the overall support of the clinical pharmacy service.
  • To act as an Independent Non-medical Prescriber, in accordance with the Health & Social Care Act 2001.
  • To liaise with primary care teams to resolve any discharge medication queries that arise.
