Support Service Provider

We are seeking a passionate and enthusiastic person with great people skills to join our Health and Local Crisis Response service as a Service Support Worker. This role will involve providing practical and emotional support to service users, along with guidance and reassurance to enable them to lead healthier and more fulfilling lives.
Key Responsibilities:
- Providing reactive, focused, and flexible support to adults following a hospital stay to enable early discharge.
- Assessing service users’ needs, completing and following a support plan for the individual.
- Supporting service users in the weeks after hospital discharge or to prevent readmission.
- Providing transportation to hospital appointments and picking up prescriptions and shopping.
Requirements:
- A full UK manual driving licence.
- A good knowledge of services provided by the NHS and Social Care.
- The ability to make things great, improving service quality for the benefit of users.
- Being professional, dealing with queries in a diplomatic and confidential manner.
- Love being flexible, working hours out with the norm suits you.
