We are seeking a highly skilled and compassionate Social Worker to join our Adult Social Care team in Sutton. As a key member of our Transition Team, you will play a vital role in supporting young adults with complex needs to live independently and achieve their goals.
Key Responsibilities- Conduct comprehensive assessments to identify eligible social care needs and develop person-centered support plans.
- Monitor and review support plans to ensure they meet the changing needs of young people and reflect good value for money.
- Safeguard and promote the welfare of young people, adhering to relevant legislation and policies.
- Manage a complex and varied caseload, making sound professional judgments with appropriate supervision and guidance.
- Collaborate with multidisciplinary teams to deliver holistic support to young people.

About YouWe are looking for a qualified Social Worker with current registration with Social Work England and a minimum of 2 years post-qualifying experience working in adult services. You should have a sound knowledge of relevant legislation, including the Care Act and Mental Capacity Act, as well as social work theories. Experience in adult safeguarding work and strong assessment, outcome-focused care planning, and risk management skills are essential.
