Job Description

As a member of our Adult Crisis Resolution and Intensive Home Treatment Team, you will be responsible for:

  • Coordinating Shifts: Ensure smooth transitions and maintain effective communication with colleagues.
  • Triage and Assessment: Conduct thorough assessments, triage referrals, and develop tailored plans.
  • Risk Management and Safety Planning: Identify and mitigate risks, develop safety plans, and collaborate with internal and external agencies.
  • Liaison and Collaboration: Establish strong relationships with stakeholders, facilitate liaison, and contribute to service development.

Requirements

To succeed in this role, you must have:

  • A current professional registration with an approved body (e.g., NMC, HCPC, or SWE).
  • Evidence of Continuing Professional Development (CPD).
  • Key skills in literacy, numeracy, and IT (Q level 2 or equivalent).
  • A post-graduate qualification in a relevant area.
  • Experience in providing clinical supervision to individuals or groups.
