Job Overview
We are seeking a qualified Mental Health Practitioner to work with our team in Greater Manchester. As a key member of our community mental health service, you will work closely with patients to understand their unique needs and develop personalized care plans.
Main Responsibilities
- Undertake comprehensive assessments of patients' mental health needs, including their medical history, current symptoms, and personal circumstances.
- Develop and implement customized care plans for each patient based on their goals, preferences, and values.
- Provide timely support and intervention during mental health crises, aiming to de-escalate situations and ensure patient safety.
- Maintain accurate and up-to-date records of patient interactions, care plans, and progress.
Key Skills and Qualifications
- Must hold a mental health nursing degree or equivalent qualification.
- Fully enhanced valid DBS check.
- Community experience is highly desirable.
Remuneration Package
- Hourly Pay Rate: £27 per hour (PAYE inclusive) / £21.68 per hour (exclusive).
