Job Title: Adult Community Inclusion Support Worker

We are seeking a dedicated and compassionate Support Worker to join our team at Precedo Healthcare. As an Adult Community Inclusion Support Worker, you will play a vital role in supporting individuals with daily living tasks and promoting their independence in the community.

Key Responsibilities:
  • Provide person-centered support to individuals, empowering them to achieve their goals and live a healthy, positive lifestyle.
  • Build strong relationships with service users, their families, and other professionals to ensure a holistic approach to support.
  • Communicate effectively with individuals, using language that is meaningful to them, and provide feedback to others as needed.
  • Advocate for individuals and support them in accessing various opportunities and services.
  • Work collaboratively with the Precedo Healthcare team to ensure a seamless and supportive experience for service users.
