About the Role

Crosby House Surgery is seeking a highly skilled and motivated Registered Practice Nurse to join our team. As a Practice Nurse, you will play a vital role in delivering high-quality nursing care to our patients and supporting the delivery of services within the practice.

Key Responsibilities
  • Provide and maintain a high standard of nursing care for patients, including routine check-ups, health assessments, and disease management.
  • Support the practice in achieving quality indicators and targets of the GMS contract and relevant National Service Frameworks.
  • Undertake male, female, and family planning assessments, provide advice, and refer patients appropriately.
  • Perform cervical screening and promote breast awareness.
  • Undertake accredited training in specific chronic disease management, such as diabetes, respiratory, epilepsy, and CHD, as required.
  • Contribute to the maintenance of the practice's disease registers.
  • Provide advice on lifestyle issues, assist and support patients in changing their unhealthy behaviors, and refer appropriately.
Person Specification
  • Registered Nurse with a valid registration.
  • Registered Smear Taker.
  • Completed Core Immunisation Training.
  • Desirable qualifications: Diabetes Course, Asthma Course, COPD Course.
  • At least 12 months post-registration experience.
