We are looking for a skilled and experienced professional to join our team as a Team Leader for the Transfer of Care Hub. The successful candidate will have a proven track record of leading and managing healthcare teams, with a strong understanding of community services and local commissioning objectives.

In this role, you will be responsible for providing day-to-day leadership and clinical expertise to the health staff within the Transfer of Care Hub. You will also work closely with other teams to co-ordinate and support hospital discharges, ensuring that patients receive timely and safe care.

Key Responsibilities

  2. Co-ordinate and support hospital discharges
  3. Collaborate with other teams to optimise patient flow

The estimated salary for this role is £52,000 - £72,000 per annum, based on your experience and qualifications. You will also have access to a range of benefits, including comprehensive training and development opportunities.
