About the Role

This position requires a detail-oriented and caring Homecare Coordinator to oversee the delivery of high-quality home care services to our clients, working closely with care teams to ensure seamless care transitions.

Key Responsibilities
  • Create and maintain detailed care plans aligned with CQC guidelines
  • Evaluate and enhance medication management processes
  • Ensure timely completion and updates of care records on our systems
  • Support the Registered Manager in achieving excellent care outcomes
  • Participate in on-call duties on a rotational schedule
