Routes Healthcare (North) is recruiting a dedicated Homecare Coordinator to join our team in Denton, Greater Manchester.
About the RoleThis position requires a detail-oriented and caring Homecare Coordinator to oversee the delivery of high-quality home care services to our clients, working closely with care teams to ensure seamless care transitions.
Key Responsibilities- Create and maintain detailed care plans aligned with CQC guidelines
- Evaluate and enhance medication management processes
- Ensure timely completion and updates of care records on our systems
- Support the Registered Manager in achieving excellent care outcomes
- Participate in on-call duties on a rotational schedule
- An annual salary of up to £23,000
- Supplemental payment for on-call responsibilities
