Mental Health Liaison Specialist

Job Overview
The Kent and Medway NHS and Social Care Partnership Trust is seeking a skilled Link Practitioner to play a key role in its Forensic Outreach Liaison Service. This 12-month pilot aims to improve discharge planning and ensure smoother transitions between services.
The successful candidate will work closely with the Community Mental Health Teams (CMHT) and Forensic Outreach Liaison Service (FOLS) teams to identify and address barriers to discharge. They will provide vital support to patients, carers, and CMHT professionals, ensuring a safe and seamless transfer of care.
Main Responsibilities
- To facilitate effective collaboration between FOLS, CMHT, and other relevant services to ensure timely discharge and transfer of care.
- To work with patients to develop personalized care plans and address any challenges or concerns.
- To communicate effectively with service users, some of whom may display challenging behaviors.
- To provide specialist clinical advice and support to multi-agency teams, promoting high-quality care for service users in the community.
- To develop strong relationships with stakeholders in criminal justice, statutory, and third sector organizations.
- To contribute to the development and progression of initiatives to improve care quality, working collaboratively with managers and clinical leads.
- To ensure that practice is informed by the best available evidence and responsive to individual service users' needs.
- To escalate any issues of poor professional conduct or practice as necessary.
