The role involves working alongside individuals who require support, helping them meet their needs and maintain their independence.

  • Supporting safe discharges from hospital
  • Ensuring individuals receive necessary support to prevent re-admissions
  • Providing short-term care-at-home services, tailoring support to individual needs
