We're seeking an experienced and compassionate Field Care Supervisor to join our team. As a Field Care Supervisor, you will play a vital role in supporting the delivery of high-quality care services to our clients in their own homes.

Key Responsibilities:

  • Support the planning and coordination of care worker rotas to ensure seamless delivery of care services.
  • Conduct regular reviews of individual services, including risk assessments and needs assessments.
  • Participate in multi-disciplinary reviews of service users' needs.
  • Shadow new care workers to ensure their competence and provide on-site observation and evaluation of care worker practice.

Requirements:

  • Experience in carrying out initial risk and needs assessments in respect of individual service users.
  • Ability to work in partnership with service users, their family, carers, advocates, healthcare professionals, and others to ensure seamless delivery of holistic packages of individual care support.
