Personalised Care Coordinator

About the Role
The successful candidate will have excellent leadership skills, with the ability to motivate and support staff to deliver exceptional care. They will also need to have a strong understanding of care standards and legislation, including the Care Standards Act. In addition, they will need to have excellent communication and interpersonal skills, with the ability to work effectively with colleagues, residents, and external agencies.
Responsibilities
• To assess, plan, deliver, and evaluate care to meet individual resident needs
• To oversee the care being provided and identify shortfalls
• To formulate, implement, and regularly maintain residential care plans
• To effectively lead the team and ensure that all contact is polite, friendly, warm, and supportive
