About the Role

This is an exciting opportunity to join our Integrated Neighbourhood Team as a Community Health Worker. In this role, you will be working alongside district and community nurses, generic workers, community matrons, occupational therapists, and physiotherapists to provide holistic care to patients in their own homes.

">

The North West Rural Integrated Neighbourhood Team has an opening for a B3 Generic Worker to work alongside the Community Nursing and/or Therapy team (depending on applicants skillset). You will be actively supported by regular supervision and individual annual appraisal.

">

You will be expected to work autonomously and on behalf of the patients served by the local area. You will have access to skills development training relevant to the post. The role involves providing nursing interventions to housebound residents in a specific geographical area who are 18 and over, registered with an Ipswich GP.

Our main aim is to support, maintain and encourage people to make healthy lifestyle choices in the ageing well process. Conditions include long-term neurological conditions, palliative and end of life care; diabetes; acute medical episodes, wound care and phlebotomy - all with a view to preventing admission and supporting early discharge.

Many of our patients have a complexity of needs across the spectrum of physical and mental and social care needs. As a Community Health Worker, you will be required to work flexibly to accommodate patient/service needs. This includes making your own travel arrangements to meet the needs of the service if required.

The successful candidate will be responsible for:

  • A QCF care certificate or equivalent knowledge and experience or working towards an Apprenticeship;">
  • Experience of working with the general public;">
  • Keyboard skills and knowledge of SystmOne;">
  • Good interpersonal skills;">
  • Able to work flexibly to accommodate patient/service needs.">

The estimated salary for this position is around £25,500 - £31,383 per annum, depending on experience.
