We are seeking a highly skilled Social Worker to join our Adults Mental Health Team as a Care Coordinator.

Core Responsibilities:

The successful candidate will conduct comprehensive assessments, develop care plans, and deliver therapeutic interventions to achieve positive outcomes for service users.

Main Objectives:

  • To undertake thorough assessments of complex care cases to inform care planning.
  • To collaborate with the multidisciplinary team to ensure high-quality service delivery.
  • To maintain effective relationships with service users, carers, and other stakeholders.
  • To stay up-to-date with best practices in adult mental health care.

Key Requirements:

  • A degree in social work (Degree/DipSW/CQSW) with at least 3 years' experience.
  • Proven experience working with older adults' mental health.
  • A valid UK driver's license is desirable.

Compensation and Benefits:

  • An estimated annual salary of £45,000 - £60,000 based on experience.
  • Parking facilities are available at or near the workplace.
