Job Summary

We are seeking a skilled and compassionate Community Support Worker to join our team. As a Community Support Worker, you will play a vital role in providing practical support and guidance to service users in their own homes or place of residence.

Key Responsibilities
  • Assist with the delivery of patient-centered care under the supervision of a senior member of the team.
  • Provide physical health checks, emotional support, carers' support, and signposting to other organizations as needed.
  • Work collaboratively with key partner organizations, service users, carers, clinicians, and other practitioners within the multi-disciplinary team to deliver services.
Requirements
  • Basic understanding of mental health conditions and how they can affect people's day-to-day lives.
  • Basic IT skills, including proficiency in Word and Outlook.
  • Experience working with people with mental health issues and vulnerable service users.
  • Ability to work effectively and flexibly without direct supervision, carrying out duties as requested.
