About the Role:

We are seeking a skilled Patient Care Navigator to join our team. As a Patient Care Navigator, you will be responsible for establishing strong relationships with caregivers and clients, managing day-to-day office and field operations, and ensuring quality assurance of services.

About the Responsibilities:

  • Develop and maintain effective communication with caregivers and clients to ensure seamless service delivery
  • Oversee daily office and field operations to guarantee quality standards are met
  • Conduct initial and ongoing in-home evaluations, caregiver introductions, and client service coordination
  • Recruit, train, and supervise a team of caregivers to deliver exceptional care
  • Take initiative to identify areas for improvement and implement solutions to meet goals

About You:

  • You have 1-3 years of experience in home care or a related healthcare industry
  • You possess strong management and supervisory skills, with a track record of success in team-oriented environments
  • You have experience with scheduling and planning, and excel at prioritizing tasks
  • You possess exceptional interpersonal and communication skills, with a talent for building strong relationships
  • You have outstanding leadership abilities and can effectively manage the demands of this role
