About the Role

We are seeking a skilled and dedicated Nurse Specialist to join our Rheumatology team. As a DMARD Nurse Specialist, you will play a vital role in providing high-quality care to patients with rheumatological conditions.

Your primary responsibility will be to monitor patients receiving Disease Modifying Antirheumatic Drugs (DMARDs) and ensure their smooth transition from secondary to primary care. You will work closely with the rheumatology consultant team, escalating any concerns to them.

Key responsibilities include:

  • Monitoring patients on DMARDs and managing their care
  • Liaising with GPs and other community services to ensure seamless care
  • Communicating effectively with patients and their families via telephone

We are looking for a candidate who is digitally literate and confident in using various digital systems to record blood results and patient communications. Full training will be provided on the required systems.
