Advanced Practice Nurse Leader

This is an exciting opportunity to join our community healthcare team as an Advanced Nurse Practitioner. You will be working with patients who have complex needs and long-term conditions, providing high-quality care and support.
Key Responsibilities- Provide skilled, effective, and evidence-based nursing care to patients with complex care needs in a variety of community settings.
- Utilise advanced clinical and diagnostic skills and expert knowledge to carry out thorough assessments of physical and psycho-social needs to develop a comprehensive short-term care plan aiming to optimise health and wellbeing.
You will be required to work within Clinical Guidelines, Policies, Procedures, and Patient Group Directions (PGDs), utilising expert judgment to work outside these frameworks where necessary. The salary for this role is estimated to be around £55,000 - £70,000 per annum.
