Community Mental Health Practitioner

Cornwall Partnership NHS Foundation Trust is seeking a dedicated and compassionate Community Mental Health Nurse to join our multidisciplinary team in North Cornwall.
We strive to deliver high-quality, effective services to individuals experiencing severe and enduring mental health problems. Our team offers assessment and treatment of mental health issues, focusing on recovery and promoting independence for our service users.
We are looking for a candidate who embodies our Trust's vision and values, is motivated to contribute to innovation and service development, and is passionate about mental health. Ideally, you will have a core profession in Mental Health Nursing and experience working independently in a mental health setting.
As a member of our team, you will need to possess a positive outlook and sense of humor to navigate the challenges of working in a busy locality that spans seaside towns and rural areas.
You will be responsible for assessing care needs and managing a defined caseload within our Mental Health Service. You will also develop and implement programs of care, utilizing our care coordination system, and foster collaboration between primary and secondary care services to provide tailored packages of care.
We require knowledge of the CPA process, mental health legislation, and The Mental Health Act. You will demonstrate a commitment to client-centered care, work flexibly as part of a team, and deliver innovative health and social care.

Person Specification- Current professional registration - NMC/HCPC
- Evidence of post-basic training
- Extensive experience of working with mental health patients
- Teaching and Assessing in Clinical Practice
- Sound working knowledge of the Mental Health Act of 1983 and other mental health legislation
- Ability to supervise staff, demonstrating the highest clinical standards
