Community Support Specialist

Cheshire East Council is seeking a skilled Community Support Specialist to join their adult social care teams. The successful candidate will have a strong background in supporting vulnerable adults and their families, with excellent communication and organizational skills.

The role involves identifying and coordinating community resources to support adults with eligible care and support needs, as well as working closely with local organizations and services to develop and improve community support.

Key responsibilities include:

  • Conducting needs-based assessments and arranging services and care for clients
  • Managing a caseload of clients and carers, providing support and guidance as needed
  • Working collaboratively with other teams and services to ensure seamless delivery of care and support

The ideal candidate will have experience working in a social care or health setting, with a strong understanding of the Care Act and its implications for adult social care. They will also possess excellent communication and interpersonal skills, with the ability to work effectively with a diverse range of stakeholders.
