Position Overview
An exceptional opportunity has emerged for a qualified social worker to become part of the dynamic Wolds Community Wellbeing team. This role involves a flexible working model, allowing for a combination of office and remote work. The team operates across a broad geographical area, necessitating a valid driving license, with the possibility of claiming mileage expenses.
Key Responsibilities
- Conduct person-centered assessments in the community in accordance with the Care Act 2014.
- Assist in the development of collaborative care and support plans, ensuring ongoing monitoring and review of services provided.
- Participate in a duty rota, responding to urgent and crisis situations as they arise.
Essential Qualifications
A solid understanding of policies, operational practices, and procedures related to adult social care is crucial. Familiarity with legislative frameworks, including the Mental Capacity Act 2005, adult safeguarding, and deprivation of liberty safeguards, is required.
