About the Role

We are seeking an experienced and dynamic lead nurse to manage the Tower Hamlet Community Diabetes Service. As a key member of the team, you will play a crucial role in providing leadership and expertise in the delivery of high-quality diabetes care to patients living with Type 1 and Type 2 diabetes.

The post holder will focus on managing the smooth running of community diabetes across Tower Hamlet Borough, working closely with GP practices and other healthcare professionals to deliver coordinated care.

The ideal candidate will have significant experience as a Band 7 DSN in the community, primary care, and/or inpatient setting. They will have a strong background in change management, service improvement, and service planning and development.

The successful candidate will be a confident and committed clinical leader who is driven by challenges and demonstrates personal resilience in a fast-moving and changing care environment.

Key Responsibilities

  • Manage the Tower Hamlet Community Diabetes Service, working closely with GP practices and other healthcare professionals
  • Provide leadership and expertise in the delivery of high-quality diabetes care to patients living with Type 1 and Type 2 diabetes
  • Develop and implement effective management plans, treatment advice, and support for all adults with diabetes
  • Contribute to the enhancement of diabetes nursing by acting as a credible role model, educator, and expert clinician
  • Collaborate with external agencies involved in providing diabetes care to Tower Hamlet patients
