About the Role

We are seeking an exceptional Palliative Care Specialist Nurse to join our team at Worcestershire Acute Hospitals NHS Trust. As a key member of our Hospital Palliative Care team, you will work closely with the CNS team to develop your clinical skills in End of Life and Specialist Palliative care delivery.

Key Responsibilities
  • Provide specialist and holistic palliative care assessments of patients and their carers' needs, formulating a plan of care using specialist knowledge and skills.
  • Work as a support nurse in palliative care across the accepted domains of the role, including clinician, educator, researcher, and manager.
  • Act as a primary resource for site-specific specialist nurses, nursing/medical teams, and the multidisciplinary team.
  • Coordinate care across different professionals and agencies, facilitating and assisting with complex discharge planning.
