Nurse Care Coordinator

An exceptional opportunity has arisen for a dedicated Registered Nurse to work in a leading care home service. You will be part of a specialist team trained in delivering high-quality care to residents with dementia and other long-term medical conditions.
The successful candidate will lead a team of care staff to deliver exceptional care, producing well-developed care plans and detailed risk assessments. They will also maintain accurate documentation and resident records, overseeing all aspects of medicine management on their shift.
The ideal candidate will have a passion for delivering great care and supporting residents and their families. They will be a team player who engages well with others, able to communicate effectively at all levels and demonstrate awareness and understanding of relevant legislation.
The successful Nurse will receive an excellent salary of £20.62 per hour and the annual salary is up to £47,178.56 per annum. They will also receive a range of benefits, including generous holiday entitlement, free meals, and ongoing personal development opportunities.
Key responsibilities include:
- Leading a team of care staff to deliver exceptional care
- Producing well-developed care plans and detailed risk assessments
- Maintaining accurate documentation and resident records
- Overseeing all aspects of medicine management on their shift
- Supporting, guiding, teaching and mentoring others in line with the NMC Code
