Under the guidance of a qualified practitioner, you will implement care plans and provide support for service users and their care givers within different community settings, and report on the service users' wellbeing.
The post holder will work flexibly in a range of settings, and in partnership with other agencies, carers/family members and the wider multi-disciplinary team.
There will be opportunities for continued professional development, including training around Positive Behaviour Support (PBS) and Trauma Informed Care.
The team works across two bases, with a focus on delivering high-quality care and support to service users.
Main Duties of the JobWorking as part of the Community Team, you will undertake short-term, focused work with the aim of maintaining the wellbeing of service users.
You will provide direct care, advice, and training to families and care providers to ensure support plans are in place, and work with other health agencies to support service users in their environments under the direction of the nursing team.
There will be an expectation that you will travel to work with people across the footprint of the trust.

Person SpecificationQualificationsEssential Criteria- Appropriate Learning Disability/Mental Health related NVQ level 2
- To demonstrate a range of work procedures and practices base level of theoretical knowledge
- NVQ level 3 in LD/Mental Health
- Clear understanding of working within a Person Centred framework
- Understanding of Positive Behaviour Support and the importance of consistency in the delivery of support and care
- Knowledge and management of clients with LD and other associated health needs
- Record keeping and report writing skills, including IT skills to manage electronic patient notes
- Ability to demonstrate how to Implement clinical care/care plans Ability to undertake observations and record these accurately
- Good computer/IT skills
- Special interests related to one or more of the teams priority areas (challenging behaviour, autism, active support, communication)
- Experience of working with people with learning disabilities in a range of settings
- Experience working with people who have challenging behaviour
- Experience of supporting people with a learning disability in planning and implementing activity programmes
- Working with a community team or other MDT.
- Experience of demonstrating modelling activities to less experienced support workers/carers.
