Early Intervention CPN Role

Hunter Gatherer Mental Health is seeking a skilled and dedicated Community Psychiatric Nurse (CPN) to join our Early Intervention team in Greater Manchester. As a key member of our team, you will work closely with our professional Community Psychiatric Nurses to provide high-quality patient care and support.

Key Responsibilities:

  • Provide mental health care and support to patients in the community
  • Work collaboratively with multidisciplinary teams to achieve patient outcomes
  • Develop and implement individualized care plans to meet patient needs
  • Conduct assessments and evaluations to identify patient needs and develop strategies for improvement
  • Provide education and support to patients and their families to promote mental health and well-being

Requirements:

  • Current and valid registration with the NMC
  • Proven experience in a CPN recovery role
  • Strong communication and interpersonal skills
  • Ability to work independently and as part of a multidisciplinary team
  • Commitment to providing high-quality patient care
