Adult Services Care Coordinator

About The RoleWe are seeking a skilled and dedicated individual to join our Combined Placement Sourcing Team (CPST) as a Care and Support Broker. This exciting opportunity is part of the health and social care system, ensuring that individuals requiring supported discharge from hospital have access to the necessary services. As a Care and Support Broker, you will work closely with our team to provide support to customers, navigating the complexities of the health and social care system.
Key Responsibilities- Support the planning and arrangement of customer-focused and strengths-based services for individuals with care and support needs.
- Accurately relay information to service providers and health and social care colleagues to enable the sourcing of appropriate services.
- Organise and prioritise work, working independently and under pressure to meet tight deadlines.
- Interpret intelligence received to determine risks and communicate safeguarding and service quality issues through relevant channels.
- Utilise excellent written, verbal, influencing, and negotiation skills to achieve successful outcomes.
- Employ sound and accurate IT and keyboard skills to manage data and information.
- NVQ level 3 in a relevant subject or equivalent qualification or experience.
- GCSE English and Maths or equivalent qualification or experience operating at this level of literacy and numeracy.
- Good up-to-date working knowledge of relevant legislations and organisational policies, including the Care Act (2014) and Hospital Discharge Guidance 2020.
- Experience of monitoring data and information, including financial materials, and using a range of IT and data systems.
- Good understanding of office systems and practices related to carrying out a health or social care role.
- Influencing and negotiating with individuals, stakeholders, and service providers (desirable).
