Homeless Outreach Practitioner Specialist

About the Role:
Aquarius is seeking a qualified Homeless Outreach Practitioner to join our team. As a key member of our team, you will work with clients who are homeless or at risk of homelessness, providing support and guidance to help them access accommodation and maintain their tenancies.
Key Responsibilities:
You will:
- Provide support and guidance to clients who are homeless or at risk of homelessness.
- Work with clients to identify and address the barriers they face in accessing accommodation.
- Liaise with voluntary and statutory agencies and health professionals to ensure clients receive the support they need.
- Maintain accurate and up-to-date records of client progress and interactions.
- Work flexibly to meet the needs of clients, including evenings and weekends.
Requirements:
To be successful in this role, you will need:
- A qualification in health/social care, youth and community work (e.g. NVQ Level 3 or above, DipSW, Mental Health Nursing, Counselling, Addiction Studies).
- Experience of supporting clients in finding accommodation and preventing homelessness.
- A flexible approach and a commitment to the principles of confidentiality.
- Excellent communication and interpersonal skills.
