Cygnet is a leading provider of specialist mental health services, and we're seeking an experienced Patient Care Manager to join our team.
Job Description
- Provide leadership and guidance to ensure effective delivery of care services
- Develop and implement person-centered care plans that meet individual needs
- Maintain accurate records and reports on patient progress and outcomes
- Collaborate with multidisciplinary teams to ensure seamless care coordination
- Monitor and improve service quality, identifying areas for growth and development
- Foster a positive work environment, promoting staff well-being and engagement
Requirements
- Degree in Social Work, Nursing, or related field
- Proven experience in healthcare management, preferably in mental health settings
- Excellent communication, interpersonal, and leadership skills
- Able to work independently and as part of a team
- Ability to adapt to changing priorities and deadlines
