We are seeking a skilled Diabetes Practitioner to join our team at Canterbury Integrated Healthcare.
Key Responsibilities- Work collaboratively with a multi-disciplinary team to provide high-quality patient care.
- Assess and triage patients, providing definitive treatment and referrals as necessary.
- Advise patients on general healthcare and promote self-management.
- Liaise with professionals, families, and carers to provide clinical information.
- Support the delivery of Enhanced Health in Care Homes, including assessment, proactive support, and timely response to in-reach teams.
- Perform specialist health checks and reviews, support anticipatory care plans, and lead certain services.
- Communicate effectively with patients, colleagues, and stakeholders to deliver a person-centred service.
- Ability to work as part of a team and communicate effectively.
- Strong clinical skills and knowledge of local and national guidelines.
- Ability to maintain accurate and contemporaneous health records.
- Excellent communication and interpersonal skills.
