Job Overview:
We are seeking an experienced Community Health Care Support Worker to join our District Nursing Service team. As a key member of our team, you will work under the direction and supervision of the Team Leader, implementing nursing care plans and contributing to the delivery of high-quality patient care.
Your duties will include performing a variety of clinical and clerical activities delegated by the Caseload holder, working in various settings such as patients' homes, clinics, GP surgeries, residential, and nursing homes. You will also be responsible for maintaining medical and surgical stock levels, adhering to agreed minimum and maximum stock levels for the District Nursing Team.
This is an excellent opportunity to join our dynamic team, providing support to the District Nursing Team in managing multi-disciplinary/multi-agency packages, ensuring an efficient and effective service delivery to our practice population.
Key Skills and Qualifications:
- Level 3 NVQ/QSF in a health care associated certificate or equivalent skills, knowledge, and experience
- Ability to work independently and as part of a team
- Experience in providing care and undertaking clinical observations
- Good communication and interpersonal skills
