About the RoleWe are seeking an experienced and motivated Clinical Lead District Nurse to join our team at Somerset NHS Foundation Trust. As a key member of our nursing leadership team, you will be responsible for providing exceptional patient care and leading by example.
The successful candidate will have significant experience in district nursing and a proven track record of delivering high-quality patient outcomes. You will be a confident leader with excellent communication skills and the ability to motivate and inspire your team.

Key Responsibilities- Lead a team of district nurses and provide guidance and support to ensure excellent patient outcomes
- Develop and implement effective care plans that meet the individual needs of patients
- Collaborate with other healthcare professionals to deliver seamless patient care
- Conduct regular audits to monitor patient outcomes and identify areas for improvement
- Significant experience in district nursing and a proven track record of delivering high-quality patient outcomes
- Leadership and management qualifications or willingness to undertake training
- Excellent communication and interpersonal skills
- Ability to work flexibly and respond to changing priorities
- A competitive salary of £42,000 - £59,000 per annum
