About the Role

We are seeking a highly skilled and motivated Practice Nurse to join our dynamic nursing team at Seaside Medical Centre. As a Practice Nurse, you will play a vital role in providing high-quality, patient-centered care to our diverse patient population.

Key Responsibilities

  • Conduct comprehensive nursing assessments and provide appropriate care and treatment in conjunction with GPs, Nurse Practitioners, and other practice nurses or independently, in line with competence and need.
  • Maintain or improve standards of care and be actively involved in identifying practice population needs.
  • Demonstrate and apply knowledge related to the Quality and Outcomes Framework (QOF).
  • Provide therapeutic interventions within a broader health promotion/public health strategy and enable patients to take action to meet their priorities for health.
  • Help provide education and training regarding chronic clinical conditions as required.
  • Provide specialist advice to patients and relatives in relation to COPD, Asthma, and, where qualified, Diabetes chronic disease conditions.
  • Undertake any further training required to ensure appropriate role development.
  • Practice within the NMC Code of Professional Conduct and maintain accurate records.

Person Specification

  • Relevant nursing/health degree.
  • Demonstrable commitment to professional development.
  • Mentoring/teaching qualification.
  • Clinical supervision training and experience.
  • Registered nurse with a desire to work in General Practice.
  • Recent primary and community nursing experience.
  • Experience of EMIS or similar clinical system.
  • Leadership Management of long term conditions.
