**Job Summary:**

We are seeking an experienced and compassionate Care Support Team Lead to join our team at Home Group Limited. As a key member of our Positive Behavioural Support (PBS) team, you will play a vital role in providing high-quality care and support to individuals with complex needs.

In this rewarding role, you will lead a team of dedicated care professionals, ensuring that every individual receives the best possible care and support. With a focus on person-centred care, you will work collaboratively with colleagues, families, and external agencies to develop and implement tailored care plans.

Key Responsibilities:

  • To manage a team of PBS Support Workers in providing high-quality care and support to individuals with complex needs
  • To lead and motivate the team to achieve exceptional outcomes and promote a culture of excellence
  • To work closely with colleagues, families, and external agencies to develop and implement person-centred care plans
  • To ensure that all aspects of care delivery meet regulatory standards and quality expectations
  • To maintain accurate records and reports, and contribute to the continuous improvement of care services

About You:

  • A proven track record in leadership and management within healthcare or social care settings
  • An understanding of positive behaviour support principles and practices
  • The ability to work effectively with individuals with complex needs and their families
  • Excellent communication and interpersonal skills, with the ability to build strong relationships
  • A patient and calm approach, with the ability to adapt to changing situations
