Role Overview

We are seeking a skilled Clinical Case Manager to join our team at Bush & Company Rehabilitation Limited. As a key member of our rehabilitation team, you will play a vital role in supporting children and young people with life-changing injuries and illnesses.

About the Role

This is an exciting opportunity for a qualified physiotherapist to take on a new challenge and develop their skills in case management. You will be responsible for managing a caseload of clients, leading assessments in their homes, and coordinating their rehabilitation plans.

Key Responsibilities

  • Manage a caseload of clients through their rehabilitation journey.
  • Lead on assessment in the client's home and set clear goals for their rehabilitation.
  • Coordinate all aspects of care and support, including accessing statutory services, therapy, and financial support.
  • Build strong relationships with clients, their families, and other professionals involved in their care.

Requirements

To be successful in this role, you will need:

  • A valid clinical registration with a relevant body (e.g., HCPC, NMC).
  • Previous experience in case management or working in a community setting.
  • Strong relationship-building skills and a willingness to travel extensively across the UK, Scotland, and Wales.
  • A competitive salary of around £45,000 - £55,000 per annum, depending on experience.
