Senior Physiotherapist

About the Role
We are seeking a highly skilled and experienced Senior Physiotherapist to join our team in a new and exciting role within our Care Home project. As a Senior Physiotherapist, you will be responsible for providing holistic patient assessments to a range of patients, working as part of a multi-disciplinary team to ensure effective and quality care for adults in care homes.
Key Responsibilities
- Provide clinical leadership and expertise to the Care Home project team, ensuring that patients receive the right care in the right place.
- Develop and implement effective therapy support plans to meet the needs of care home residents, working closely with care home staff and other healthcare professionals.
- Build and maintain effective relationships with care home managers and owners, promoting the role and its benefits.
- Work closely with social care providers, primary care, and commissioning teams to ensure seamless care and effective outcomes for adults.
- Develop and gather information to demonstrate the effectiveness of the Care Home project, including the production of regular monitoring reports.
Requirements
- Proven post-registration experience as a physiotherapist, with a range of rotational acute/community care experiences.
- Ability to work as an autonomous practitioner, with excellent clinical skills and communication skills.
- Experience of working with assistants/technicians and building effective working relationships with patients/carers/colleagues.
- Directing patient care and experience of evidencing and auditing practice.
