Job Overview

The York and Scarborough Teaching Hospitals NHS Foundation Trust is seeking a highly skilled and motivated Geriatrician to lead the development of the Frailty Virtual Ward. This innovative service aims to provide acute care to patients in their own home environment, supporting early hospital discharge and admission avoidance.

The successful candidate will be responsible for assessing patients, devising holistic management plans, and working collaboratively with the multidisciplinary team to achieve optimal patient outcomes. The role requires strong communication and interpersonal skills, with the ability to establish good working relationships with patients, carers, and colleagues.

Main Duties of the Job

The post holder will be expected to:

  • Assess patients in their own home environment
  • Work collaboratively with the multidisciplinary team to devise holistic management plans
  • Triage referrals and perform comprehensive geriatric assessments
  • Create treatment plans and refer to specialty colleagues as appropriate

Person Specification

The successful candidate will possess:

  • A full UK driving licence and access to their own vehicle
  • Strong communication and interpersonal skills
  • Experience of working in multidisciplinary teams
  • Able to establish good working relationships with patients, carers, and colleagues

Qualifications and Experience

The successful candidate will hold:

  • A MBBS or equivalent medical qualification
  • ALS training or equivalent
  • Evidence of NHS employment in a training or service post
  • Satisfactory completion of Foundation Training or equivalent
  • Full registration with the GMC at time of starting the post
