Primary Care Specialist

As a Primary Care Specialist, you will be responsible for providing high-quality primary care services to patients in their own homes and in our clinics. You will work closely with our multidisciplinary team to ensure that patients receive comprehensive and coordinated care.
Key Responsibilities:
* Assess and treat patients in their own homes and in our clinics
* Triage and treat patients, making referrals to other members of the primary healthcare team as needed
* Ensure clinical practice is safe and effective, and remains within the boundaries of your competence
* Advise patients on maintaining their health and wellbeing, and refer to other team members as needed
* Communicate effectively with patients, families, and other healthcare professionals
* Maintain accurate and contemporaneous healthcare records
Person Specification:
* HCPC registration
* QOF knowledge and experience
* Knowledge of primary and community healthcare
* Excellent communication and interpersonal skills
* Good organisational and IT skills
* Ability to manage and prioritise workload
* UK driving license and business insurance for home visiting
Desirable qualifications and experience include experience of SystmOne/EMIS clinical systems and working in a GP practice/primary care setting.
