About the Role

We are seeking a highly skilled and compassionate Mental Health Liaison Worker to join our team at Black Country Job Support. As a key member of our Urgent Care services, you will play a vital role in delivering a comprehensive 24/7 Liaison Mental Health Service for individuals experiencing mental health crises.

Key Responsibilities
  • Provide evidence-based mental health and risk assessments for individuals presenting with acute mental health issues and/or who have self-harmed.
  • Respond to referrals from emergency departments, acute medical units, and wards within designated timescales.
  • Undertake comprehensive biopsychosocial/risk assessments and develop care packages for patients meeting criteria for support from local mental health services.
  • Ensure referral and access to other services align with patient needs, agreement, or best interests.
  • Utilize mental health assessment skills to reduce duplicate assessments and signposting.
  • Record interventions on electronic patient record systems and maintain accurate, clear, and organized documentation.
  • Provide information, education, support, and advice to patients and their families/carers, including communicating complex and sensitive information.
  • Support acute trust colleagues providing care to patients with mental health problems.
  • Recognize and interpret abnormal signs/symptoms, including physical symptoms and underlying physical conditions impacting mental health needs.
Requirements
  • Experience working in Urgent Care services, including crisis services, inpatient wards, and liaison services as a registered nurse.
  • Ability to complete biopsychosocial assessments and risk assess/implement safety plans.
  • Capacity to prioritize work and demonstrate continued professional development.
  • Previous experience in a liaison service is desirable.
