Job Title: Care Navigator Administrator

We are seeking a highly organized and detail-oriented Care Navigator Administrator to join our team at The Links Medical Practice. As a Care Navigator Administrator, you will be the first point of contact for patients, providing initial support with queries, including compliments and complaints. You will also support clinicians by liaising with patients regarding appointments, referrals, registrations, general inquiries, and more.

Main Duties:
  • Process and effectively signpost patients to the appropriate healthcare professional depending on the presenting condition.
  • Answer incoming phone calls, transferring calls or dealing with the callers' requests appropriately.
  • Be helpful and friendly to patients who make contact with the practice by phone or in person, providing the best possible patient service.
  • Make appointments for patients, take requests for home visits, and answer queries in a friendly and helpful manner.
Administrative Duties:
  • Process registering new patients, making up notes and entering their details on the computer system.
  • Process new arrivals and deductions.
  • Support with GP Links.
  • Management of the generic inbox.
Person Specification:
  • Experience of working in a primary care environment.
  • Experience of working with the general public.
  • Experience of working in a healthcare setting.
Personal Qualities:
  • Polite and confident.
  • Flexible and cooperative.
  • Motivated.
  • Problem solver with the ability to process information accurately and effectively.
Qualifications:
  • Educated to GCSE level or equivalent.
  • Active signposting or Care Navigator training qualification.
  • Healthcare qualification (level 2) or working towards gaining equivalent level.
Skills:
  • Excellent communication skills (written and oral).
  • Clear, polite telephone manner.
  • Effective time management (planning & organizing).
  • Ability to work as a team member and autonomously.
