Community Case Management Officer
We are looking for a dedicated and compassionate individual to fill the position of Community Case Management Officer in Colwyn Bay.
The successful candidate will have experience of delivering case management, support, and person-centred interventions.
Job Summary:
This is a temporary role that requires the ability to deliver case management, support, and person-centred interventions for individuals and households at risk of homelessness.
Main Responsibilities:
- Provide case management, support, and person-centred interventions for individuals and households at risk of homelessness.
- Work in collaboration with other professionals and agencies to provide effective support services.
- Maintain accurate records and reports of cases managed.
Requirements:
- Experience of delivering case management, support, and person-centred interventions.
- Ability to work in a fast-paced environment and manage multiple cases simultaneously.
- Strong communication and interpersonal skills.
Estimated Annual Salary: £32,742 - £41,787
