About the Role
Cygnet Health Care is seeking a dedicated and compassionate Care and Support Specialist to join our team at Cygnet Cedars. As a Care and Support Specialist, you will be responsible for providing high-quality care and support to individuals with learning disabilities and complex needs.
Key Responsibilities
- Provide person-centered care and support to individuals with learning disabilities and complex needs
- Develop and implement individualized care plans to promote independence and well-being
- Work collaboratively with the multidisciplinary team to ensure seamless care delivery
