About the Role

Hays Specialist Recruitment Limited is seeking a dedicated and compassionate Care Coordinator to join our dynamic healthcare team in Tameside. As a Care Coordinator, you will play a vital role in coordinating and implementing care plans for patients, liaising with other healthcare professionals, and ensuring that patients receive the highest standard of care.

Key Responsibilities

  • To act as a Care Coordinator in Tameside, responsible for the coordination, commissioning, and personal delivery of individual packages of care for a caseload of service users within Community Mental Health Teams (CMHTs).
  • To implement a plan of care for the service user in collaboration with the multidisciplinary team, referring agent, family/carer, and service user, where appropriate.
  • To assess and monitor risk, ensuring the safety and well-being of service users.
  • To offer initial mental health and social care assessments to those newly referred to the service.
  • To support the CMHT's duty system on a rota basis.

Requirements

  • Registered Nurse with valid NMC registration.
  • Experience of supporting people, their families, and carers in a related role or as a Care Coordinator.
  • Experience of data collection and providing monitoring information.
