About the Role:
We are seeking a skilled and dedicated Hub Support Worker to join our team at Engage Leeds. As a key member of our small, friendly team, you will play a vital role in processing and triaging referrals, offering initial advice, and delivering short-term prevention work.
Main Responsibilities:
- Support the delivery of the Engage Leeds city-wide housing support service.
- Process and follow up referrals delegated by the Senior Hub Co-ordinator and Hub Team Leader.
- Deliver triage interventions and signposting for people being referred and for referring agencies.
- Undertake needs and risk assessments to ensure safe practice in referral, triage, and community-based interventions.
- Carry out short-term advice/prevention work around housing support issues.
- Liaise with colleagues across Engage Leeds to ensure the provision of high-quality and timely support and advice.
- Update and maintain accurate records within the Gateway case management system.
Requirements:
We are looking for someone with excellent organisational skills, a great eye for detail, and the ability to handle various queries and conversations with confidence and empathy. You must be able to manage your time effectively and use your own initiative.
