About the Role

We are seeking a dedicated and compassionate Support Practitioner to join our team at Cornerstone. As a Support Practitioner, you will play a vital role in supporting individuals with various learning and/or physical disabilities to maintain their independence and quality of life.

Key Responsibilities

  • Support individuals with daily living tasks, including meal preparation, housework, medication management, and personal care.
  • Assist with transportation and accompany individuals on outings and activities, such as swimming, walking groups, and arts and crafts.
  • Develop and maintain positive relationships with individuals, their families, and other stakeholders.
  • Contribute to a collaborative and supportive team environment, promoting a culture of respect and inclusivity.
  • Participate in training and professional development opportunities to enhance skills and knowledge.

About You

We are looking for an individual who is passionate about providing high-quality care and support. You will possess excellent communication and interpersonal skills, with the ability to work effectively in a team. A full clean driving license is desirable, but not essential.
