Harmony Senior Services is seeking a dedicated Clinical Care Specialist to join our team at Harmony at Brentwood.
Estimated Salary: $65,000 - $85,000 per year
Job Description:
The Clinical Care Specialist will provide quality nursing care and services to residents in a respectful and dignified manner, ensuring the delivery of care and services that meet their unique needs and preferences. This role requires strong communication and interpersonal skills, as well as the ability to work effectively in a team environment.
Responsibilities:
- Coordinate physical, social, and emotional aspects of assisted living care and services in accordance with physician's orders and resident choices
- Execute medication management plans, including periodic review of documentation and observation of effectiveness
- Assess resident needs and update relevant parties as necessary
- Implement measures to ensure resident wellness and safety, documenting all actions taken
- Work collaboratively with other departments to meet resident needs
- Respond promptly and calmly to emergencies
- Report incidents and changes in resident condition to Healthcare Coordinator and document accordingly
- Participate in community events and activities
- Maintain high standards for record-keeping and staff documentation
- Establish and maintain positive working relationships with all departments
- Identify risk factors and initiate interventions, documenting results
- Provide thorough reports of resident status to oncoming nursing teams
- Support and motivate CNA/RMA staff members
- Attend required trainings, CEU's, and staff meetings
- Acknowledge and adhere to company policies and procedures
- Prioritize proper body mechanics and safe resident transferring techniques
- Acknowledge attendance policies and report to work on time
- Complete assigned duties efficiently
Requirements:
- Current RN or LPN licensure in good standing with state Board of Nursing
- Experience working with seniors, preferably in assisted living or LTC
- Demonstrated teamwork and leadership abilities
- Critical thinking and effective verbal and written communication skills
- Ability to manage time and prioritize tasks efficiently
- Good physical condition, able to sit, stand, bend, kneel, walk, and lift up to 50 lbs
- Flexibility to work an 8-16 hour schedule, including weekends and holidays
- Attendance at required trainings, CEU's, and staff meetings
