About the Role

We are seeking a skilled and compassionate Health and Wellbeing Practitioner to join our team at HMP Coldingley. As a key member of our multidisciplinary team, you will work closely with service users, prison staff, and healthcare providers to deliver high-quality health and wellbeing services.

Key Responsibilities
  • Manage a caseload of service users with low-level mental health needs and/or substance misuse issues
  • Provide harm reduction advice and guidance, conducting assessments and formulating and delivering care plans
  • Deliver structured 1:1 sessions, facilitate group-work sessions, and perform second signatory duties
  • Develop and implement release plans, ensuring transitional support for service users leaving the prison
  • Contribute to the development and implementation of service targets and measuring the effectiveness of our work
Requirements
  • Knowledge of substance misuse and low-level mental health issues
  • Demonstratable experience of carrying out risk assessments, comprehensive assessments, and designing and implementing SMART care plans
  • Significant experience of carrying out psychosocial interventions, including motivational interviewing techniques
  • A thorough knowledge of and commitment to the 12-step process of recovery from addiction
  • Experience of providing counselling/CBT services to prisoners or those with health and wellbeing issues
